Hi guys

We run one of the largest PT forums on the web and have been waiting for vbulletin for ages to answer our request for a paid for customization.

I have a section called Patient Corner and in that I wish to add a button that forces a user to post a new thread in a standard format with various fields that they must complete. This will help us to better server there needs.

It could be as a totally new button which I would activate or not for all forum categories and subcategories and then could sit along side the new thread button call "New Physio Problem"

This button would have sections to fill out e.g. fields for

History of presenting problem

Time since problem began

An irritibility scale e.g. 0 - 10 on a sliding scale
An investigations (Xray, MRI etc) findings field
A current treatment section

A 24hr behaviour section (worse in am, during the day, at night)?

You get the drift.

If you might be able to create such a mod for me then I could explain in more details the required fields.

Also I would like to customise the layout of that first thread so it looks more like a case presentation from a patients noted when people view it

This is indeed possible and I've some similar modifcations for clients in the past. I'll include more details in my email back to you about the design/skin
